Emily has been trapped in a barn with a car turned on and is knocked out by the fumes, which is most likely the cause of her hallucination/Ali sighting. The clue left behind by Ali? How did Emily get out of the barn...

Season 2, Episode 24 - If These Dolls Could Talk
Alison comes and talks to Spencer when Spencer is sleeping on the couch, the most likely cause of Spencer's Ali sighting is a dream. The clue left by Alison was a missing cap on the bottle of painkillers.

Season 3, Episode 1 - It Happened That Night
Mona has this hallucination when she is in Radley, could possibly be a true hallucination caused by her drugs.
Season 3, Episode 16 - Misery Loves Company
Alison comes to visit Aria when she is sick. The hallucination could have been caused by the drink that Merideth was feeding Aria, which was drugged. This hallucination seems real because Alison warns Aria about the drink being drugged and tells her that Merideth is after the pages.
Season 3, Episode 23 - I'm Your Puppet
Spencer sees Alison at Radley. The hallucination could have been caused by Spencer's imagination because at the time she was under a lot of stress. Alison plays the record I'm your puppet and they dance together. Alison then shows Spencer where the finish of Mona's map was.

Season 3, Episode 24 - A Dangerous Game
Hanna and Mona see Alison pull them out of the fire. The hallucination could have been caused by the smoke and the lack of oxygen, but Spencer didn't have any reason to hallucinate and she saw Alison as well.
